    Development of supplemental nutrition care program for women, infants and children in Korea: NutriPlus+

    Onto the world-fastest ageing of society, the world-lowest fertility rate prompted a development of various policies and programs for a betterment of the population in Korea. Since the vulnerability of young children of low socio-economic class to malnutrition was clearly shown at the in-depth analysis of the 2001 Korea National Health and Nutrition Examination Survey data, an effort to devise supplemental nutrition care program for pregnant/breastfeeding women, infants and preschool children was initiated. The program was designed to offer nutrition education tailored to fit the needs of the participants and special supplementary foods, using USDA WIC program as a benchmark. Based on the dietary intake of those age groups, target nutrients were selected and their major food sources were searched through nutrient content of foods and dietary pattern analysis. As a result, we developed 6 kinds of food packages using combinations of 11 different food items. The amount of each item in a food package was determined to supplement the intake deficit in target nutrients. Nutrition education in NutriPlus+ aims to improve the nutrition knowledge, attitude, and dietary behaviors of the participants, and is provided through group lessons, individual counseling sessions and home visits. Breastfeeding is promoted with top priority in education for the health of both mother and baby. The eligibility guidelines were set for residency, household income, age, pregnancy/breastfeeding and nutritional risk such as anemia, stunting, underweight, and/or inadequate nutrient intake. Income eligibility was defined as household income less than 200 percent of the Korean poverty guidelines. A pilot study to examine the feasibility of program implementation was run in 3 public health centers in 2005 and expanded to 15 and 20 in the following 2 years. The result of 3-year pilot study will be reported separately along with the ultimate nationwide implementation of the NutriPlus+ in 2008

    Development of a Research-Based Hospital Model in Korea

    Noting the increasing public attention on healthcare, Korean society has shown greater attention to the significance of the health technology (HT) development. In order to promote HT competitiveness, the role of research-based hospitals (RBHs), in producing new ideas as well as utilizing final outcomes, has grown increasingly significant. Despite high quality healthcare professionals, state-of-the-art equipment, and well-developed information technology, few hospitals in Korea are successful leaders in HT development. In order to understand HT research and development (R&D) programs in Korea as well as hospital-based R&D investment performance, this paper has analyzed a recent three-year R&D investment of the Korean government. In addition, a survey on how to promote RBHs in Korea has been proceeded through adopting the Delphi method. Several model cases of RBHs abroad have also been studied to understand key success factors in formulating a development model of RBHs in Korea. This paper proposes suggestions for the promotion of RBHs in Korea: systematic reform related to the hospitals, reinforcement of the infrastructure of the hospitals, empowering human resources and policy framework to support the hospitals

    Estimated dietary intake and risk assessment of polychlorinated dibenzo-p-dioxins and dibenzofurans and dioxin-like polychlorinated biphenyls from fish consumption in the Korean general population

    We analyzed 17 polychlorinated dibenzo-p-dioxins and dibenzofurans (PCDD/Fs), and 12 dioxin-like polychlorinated biphenyls (DL-PCBs) in samples from various fish species available at food markets in nine Korean cities. The estimated dietary intake of these chemicals was calculated from the raw concentrations of PCDD/Fs and DL-PCBs in each sample and from the food consumption of the Korean general population, and a comparison was made with the provisional tolerable monthly intake (PTMI). The average daily dietary exposure and the 95th percentile of intake of PCDD/Fs and DL-PCBs were 0.21 and 0.49 pg WHO05-toxic equivalents (TEQ) kg-1 body weight d-1 representing 5.27% and 12.26%, respectively, of the Korean tolerable daily intake (TDI). We applied the monthly fish consumption limits to the evaluation of improved risk assessment and concluded that unlimited consumption of most fish species does not contribute to the elevated cancer risk. This investigation was the first such large-scale study in Korea, and incorporated 37 species, including a species of whale, and 480 samples.     Hexabromocyclododecane (HBCD) in the Korean food basket and estimation of dietary exposure

    1,2,5,6,9,10-hexabromocyclododecane (HBCD) is a brominated flame retardant that is used worldwide in expanded and extruded polystyrene foam and simultaneously emitted to the environment. HBCD can easily accumulate in animals and humans and cause neurotoxicity, thyroid hormone disruption, and reproductive disorders. Therefore, it is essential to monitor the HBCD concentrations in foods and estimate the human exposure through the diet. In this study a total of 521 food samples from eight food categories were sampled and analyzed for their HBCD content. Based on consumption data, the average dietary intake of the general Korean population and specific subgroups was calculated. The highest levels of HBCD were found in fish and shellfish (0.47 ng g(-1) ww), and this was attributed to natural exposure to the contaminated marine environments and the HBCD bioaccumulation. In addition, the use of expanded polystyrene buoys in aquaculture was also found to be a potential source of HBCD in bivalves. The high solubility of alpha-HBCD in water and the persistence of this diastereomer compared to beta- and gamma-HBCD in biological tissues, led to higher accumulation of alpha-HBCD in animal -based foods. In contrast, the diastereomeric selectivity and lower metabolic capacity in plants compared with animals led to the predominance of gamma-HBCD in plant-based foods. The dietary intake of HBCD was estimated to be 0.82 ng kg(-1) bw d(-1) in the general population and 2.89 ng kg(-1) bw d(-1) in children up to 5 years of age.
